Depending on your reason for changing your name, you should only need your marriage certificate, deed poll document or certificate of divorce (decree absolute /decree nisi) as evidence of your new name.  Your marriage certificate is accepted where you take your husbands surname.  Many companies will also accept the wedding certificate as evidence of the bride's double barreled surname.
